At Creative Solutions in Healthcare it's our goal to formulate creative solutions in helping you to qualify for Medicare, Medicaid and other types of funding options. Please contact one of our facilities to speak with an admissions coordinator who can offer advice regarding your funding options. There are multiple ways to pay for senior housing or long term care. We gladly accept:

  • Private
  • Medicaid
  • Medicare

  • Physician office visit notes (if available)
  • Recent history and physical
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • If recent hospitalization, information from that stay to include history and physical, nurse's notes, therapy notes and discharge summary
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives
  • History and physical
  • Nurse's notes, therapy notes, lab results and current medical treatment records
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • All of this information can be facilitated with the Skilled Care Center through the discharge planner at the hospital
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives
  • Recent history and physical
  • Current nurse's notes
  • Recent hospital discharge summary (if available)
  • Facility Physician orders
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives.
  • Social Security card
  • Birth certificate and any other type of identification, such as a driver's license or Medicare card
  • Proof of all income
  • If you receive Social Security, your Social Security award letter
  • Information about cash on hand, or in bank accounts, bonds, stocks, life and/or health insurance
  • Information about property you own or are buying
  • Information regarding any monies or properties you have recently transferred
  • Information about medical treatment and medicines you need regularly
  • Medical bills you owe

Funding Options
Medicaid Funding

As defined in Title XIX of the Social Security Act, Medicaid is a joint Federal-State program which pays for medical services to eligible needy and vulnerable families and individuals. Medicaid currently pays for 60% of nursing facility care.

Medicare Funding
Admission

As defined in Title XIX of the Social Security Act, Medicaid is a joint Federal-State program which pays for medical services to eligible needy and vulnerable families and individuals. Medicaid currently pays for 60% of nursing facility care.
