Information needed for review to admit someone from home

  • Physician office visit notes (if available)
  • Recent history and physical
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • If recent hospitalization, information from that stay to include history and physical, nurse's notes, therapy notes and discharge summary
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives


Information needed for review to admit someone from a hospital

  • History and physical
  • Nurse's notes, therapy notes, lab results and current medical treatment records
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • All of this information can be facilitated with the Skilled Care Center through the discharge planner at the hospital
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives


Information needed for review to admit someone from another facility

  • Recent history and physical
  • Current nurse's notes
  • Recent hospital discharge summary (if available)
  • Facility Physician orders
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives

CREATIVE SOLUTIONS TO PAY HEALTHCARE...

Medicaid Funding

  • As defined in Title XIX of the Social Security Act, Medicaid is a joint Federal-State program which pays for medical services to eligible needy and vulnerable families and individuals. Medicaid currently pays for 60% of nursing facility care.

Medicare Funding


  • As defined in Title XVIII of the Social Security Act, Medicare ("Health Insurance for the Aged and Disabled") is a Federal health insurance program for aged (65+) and certain disabled individuals regardless of income. Please contact one of our facilities to learn if you qualify for Medicare funding.

Medicare is the federally funded program a person automatically becomes eligible for at the age of 65. It provides very limited coverage in a skilled nursing facility. Medicaid is a state funded program, with each state determining eligibility through specific criteria, which may vary from state to state. Specific coverage under the Medicaid program will be provided to eligible recipients prior to time of admission.

The following are a list of items you should have available for review at the time of application:

  • Social Security card
  • Birth certificate and any other type of identification, such as a driver's license or Medicare card
  • Proof of all income
  • If you receive Social Security, your Social Security award letter
  • Information about cash on hand, money in checking and savings accounts, savings bonds, current value of stocks, life insurance and health insurance
  • Information about property you own or are buying
  • Information regarding any monies or properties you have recently transferred
  • Information about medical treatment and medicines you need regularly
  • Medical bills you owe
