Creative Solutions in Healthcare Expands Texas Footprint with Acquisition of Five Skilled Nursing Facilities
Fort Worth, Texas, September 1, 2025—
Creative Solutions in Healthcare, the largest skilled nursing operator in Texas, has acquired operations of five skilled nursing facilities from Southwest LTC, effective September 1. This expansion brings the company’s statewide footprint to 179 properties, including 157 skilled nursing locations.
The newly acquired facilities are located in Corsicana, Jacksonville, and Gainesville. With this acquisition, Creative Solutions strengthens its presence in Corsicana and Jacksonville, where it already serves residents, while also bringing its care model to the Gainesville community.
In addition, Creative Solutions has completed the real estate acquisition of Marine Creek Nursing and Rehabilitation through a $9.5 million bridge-to-HUD loan with lending partner Oxford.

As part of the acquisition, Creative Solutions will make immediate investments into the new facilities, including replacing 100% of resident mattresses, an estimated 400 beds, and upgrading furniture and art packages. Each facility will also undergo improvements, with larger renovations assessed and planned where needed.
