What types of Rehab do you offer?

Our skilled therapists provide personalized programs to help residents attain the highest levels of independence. Trained and licensed therapists provide speech, occupational, and physical therapy to the residents.

When is Skilled Care needed?

Skilled Nursing facilities provide a higher level of medical care and support for seniors with complex medical needs. Residents in Skilled Nursing facilities often have chronic health conditions, disabilities, or post-surgery recovery needs that require regular medical attention. Skilled Nursing staff can give medications, manage wounds, and provide around-the-clock care to ensure residents’ health and safety.

If you or your loved one experience any of the following, skilled nursing may be right for you:

Am I covered by Medicare or Medicaid? What’s the difference?

Understanding your coverage is essential. Medicare and Medicaid are both government programs, but they serve different purposes. It generally covers hospital stays, doctor visits, and other medical services.

Eligibility and benefits can vary by state, and it covers a broader range of services than Medicare. Medicaid often covers long-term care services, including nursing home care, for those who qualify based on income and other criteria.

Admisiones

What documentation is required for the admission process?

Prior to the day of your admission our facility will need important documents to manage your specific transition. This will include both medical and financial information that is necessary to ensure our facility is a good fit for you.

Secure E-Form Submission Process

To protect your security, we will send our admission paperwork using a two-factor authentication method meaning that it will require both an email address and a cell phone number. The paperwork will be sent via DocuSign to your email so that you can complete it at your convenience. This will only be used to authenticate these forms and will never be shared or used for marketing purposes.

Do I need prior authorization to be admitted to one of your facilities?

Your insurance company may require prior authorization before your skilled nursing stay.

What do I need to bring with me on the day of my admission?

We have a checklist for this!

On the day of your arrival, we will aim to provide a smooth transition to our facility. As our building will be home for you or loved one, we want them to feel as comfortable as they possibly can.

We will provide the basic necessities, but we find residents feel more comfortable with some of their items from home. Here is a recommended list of things they may want to bring:

  • Photos or a photo album
  • Hairbrush and standard toiletries
  • Their favorite blanket and pillow
  • Their favorite perfume
  • Comfortable clothes, pajamas, and clothes they like to wear to events- laundry service is provided
  • Favorite music or movies
  • Assistive devices (Cane, walker, wheelchair, hearing aids, etc.)

Are residents allowed to decorate their space?

Yes, residents are allowed to decorate their space. We want our residents to feel as comfortable as possible in their new home. Residents may bring artwork, photos, their favorite color pillows and blankets, small plants, and any other items that will help them create a more personalized and enjoyable space.

We recommend leaving valuable or highly sentimental pieces at home. We do not want any precious items to be misplaced!

Our Creative Difference

Our 3 White Glove Inspectors visit at minimum twice per year to ensure that the buildings are meeting our high standard of Creative Solutions Clean. The White Glove team reviews the building inside and out with a special focus on our kitchens. They grade on a scale from 0 -100. They identify opportunities for improvement and provide training and support to our skilled Environmental Services Supervisors.
What is your Appetizer Program?
Our Appetizer Program is a unique initiative aimed at promoting nutrition and well-being among our residents. Through this program, our activities department innovatively crafts a variety of appetizers to be served before meals. This not only adds a creative and enjoyable element to the dining experience but also serves a crucial purpose in maintaining the health and sustenance of our residents.
What Activities do you offer?
Our activities department works closely with residents to ensure that the activities offered contribute to a fulfilling and enjoyable daily life. These activities are crafted to promote physical, mental, and emotional health. Residents can engage in group exercises, arts and crafts, music and dancing, and cognitive games to stimulate the mind. We also organize social events, outings, and communal gatherings to create a sense of community and connection.

What is your meal structure?
Our mealtimes and dining structure varies by facility. All of our Skilled Nursing Facilities serve three meals a day, with appetizers as well. Residents are encouraged to dine in our communal dining areas; however, exceptions can be made for residents who prefer to remain in their rooms during meal times.

The facility recognizes the importance of staff professional development. To promote staff development, the company has implemented an education assistance program for staff that has been with the company for twelve consecutive months and work at least 30 hours per week. For our currently employed LVNs that have committed to continue their studies toward an RN degree, we offer an exemplary incentive to complete the next level of their education.
Do you offer tuition reimbursement?
The facility recognizes the importance of staff professional development. To promote staff development, the company has implemented an education assistance program for staff that has been with the company for twelve consecutive months and work at least 30 hours per week. This monetary assistance may be used to offset costs incurred while seeking continued education from an accredited college or university.
