Choosing a long-term care or skilled nursing facility for yourself or a loved one can be intimidating, and you want to ensure that you are making an informed decision. At Creative Solutions in Healthcare, we understand the importance of this choice, and we are fully committed to ensuring a smooth and transparent admission process. To help walk you through what this may look like, we have highlighted the key steps below.

Starting the Admissions Process

The first step in the admission process is a referral. Our facility will need important documents to manage your specific admission process. This will include both medical and financial information that is necessary to ensure our facility is a good fit for you.

Medical Documents

Once the referral process begins, our clinical team will carefully examine your medical records. These records might include information from your primary care physicians, hospital discharge summaries, and other relevant documents. Our goal is to fully understand your medical history so that we can provide the appropriate level of care for your unique needs.

Financial Documents

In addition to reviewing your medical documents, we will also look into your insurance and financial information. Our team of experts will verify your coverage and benefits. Our goal is to be transparent with you about the cost breakdown so that you can feel fully informed in your decision.

Resident Choice

In most cases, the hospital will provide you with a list of nearby skilled nursing facilities. From there, you are free to contact each of these locations to find the option that works best for you. If you have a specific facility in mind that is not listed, you can always request that your case manager share your medical information with that facility directly. You should always have a choice in your care, and we are here to help you explore all your options.

Visiting the Facility

After reviewing your referral information, we encourage you and your loved ones to take a tour of our facility. During this visit, we showcase the services we offer and what life is like in our facility. We also take this opportunity to address any questions or concerns you may have. Your needs and goals are important to us, and we encourage you to share them with our team.

Like each resident, each admission is unique. We highly recommend taking this tour and connecting with our team, so we can better address your specific needs and tailor the admission process accordingly. Our goal is to make you feel comfortable and empowered in your decision.

If you cannot visit in person, we can provide a virtual tour or have a phone call to discuss this information.

NOTE: Coming in for a tour or calling our facility is for your benefit. We will never pressure you to commit or make any important decisions on the spot.

Admission Approval and Paperwork

Once we know we can meet your clinical needs and have verified your medical and financial information, we will begin gathering your admission forms and scheduling your transfer to our facility.

We recommend having all necessary paperwork completed prior to your arrival. To make completing the forms simple and convenient, they will be sent and submitted electronically, allowing you to complete them from your phone, tablet, or computer.

Secure E-Form Submission Process

To protect your security, we will send our admission paperwork using a two-factor authentication method meaning that it will require both an email address and a cell phone number. The paperwork will be sent via DocuSign to your email so that you can complete it at your convenience. This will only be used to authenticate these forms and will never be shared or used for marketing purposes.

Making the Transition

We will work closely with your hospital or care provider to set up and schedule transportation to our facility according to your medical needs. While the hospital is coordinating your discharge, we will be preparing your room for your arrival and working to make sure you experience a warm welcome. 

We want your new environment to feel as much like home as possible. Upon your arrival, you will be welcomed by our caring team members. Within the first few days you will also meet with each department head including our Dietary Manager, Activity Director, and Director of Nurses. They will learn about your interests, preferences, and goals to ensure every aspect of your care is tailored to your needs.
